Entry Form – 15th Annual Golf Tournament

DATE: October 10, 2010 (Note: Eagles are away and play at 8:10 p.m.)

LOCATION: White Oaks Golf Course

 Four-Man Scramble Format (same as last year). Shotgun start at noon (register at check-in table by 11 a.m.). Prizes for 1^st place, Closest to the Pin, and Longest Drive. There also will be door prizes at the buffet following the tournament right at White Oaks.

 This tournament is a fundraiser for the SJWHFO Scholarship Fund. Your donation of $100.00 per player goes toward greens fee, cart, buffet dinner (including dessert), beer, soda and prizes – and proceeds will help us fund the $1,000.00 scholarship grants to be presented at the Regions 7-8 banquet each March and help underwrite our annual free wrestling clinic (among other projects).

 Entry deadline is October 5th; only 120 spots available. Please designate the leader of the foursome (with a home phone number) so we can communicate.

If you don’t play but want to help, hole sponsorships are available at $50.00. We also need donated door prizes.
